What You'll Do

Manage a patient panel with a proactive, resolution-driven approach to address both clinical and non-clinical care gaps.

Identify and coordinate community resources that can enhance patient care.

Provide education and health literacy to patients on managing chronic conditions.

Conduct medication management, including reconciliation, adherence monitoring, and refill coordination.

Ensure timely access to essential services, such as Home Health, DME, and Home Infusion.

Foster an open and supportive dialogue with patients to understand and address their concerns.

Assist with referrals and appointment scheduling to keep patients on track with their care plans.

What We're Looking For

Active Multi-State/Compact License (LPN/LVN)

Proficiency in Microsoft Office Suite and ability to navigate electronic health records.

Strong written and verbal communication skills.

Preferred Qualifications (Nice to Have, but Not Required)

Epic EHR experience

Bilingual proficiency

Additional state-specific LPN licensures

Remote Work Requirements

Reliable high-speed internet connection

A HIPAA-compliant workspace in your home

Physical Requirements

Ability to sit for extended periods while working on a computer

Strong listening and communication skills, especially over the phone

Ability to take clear and thorough notes in real-time
