What You'll Do

Manage patient care - Monitor and support patients with chronic conditions, closing gaps in clinical and non-clinical care.

Connect patients to resources - Identify and coordinate beneficial community resources.

Educate and empower - Provide health education and guidance to help patients better manage their conditions.

Support medication management - Reconcile medications, ensure adherence, identify concerns, and coordinate refills.

Facilitate timely care - Assist with scheduling home health services, DME, home infusion, and other essential care needs.

Resolve patient concerns - Foster open communication to address questions and improve care experiences.

Manage referrals and appointments - Assist with scheduling and ensuring continuity of care.

What You Need

Active Multi-State/Compact LPN or LVN License (NLC)

Technical proficiency - Comfortable using Microsoft Office Suite and other digital tools.

Strong communication skills - Excellent written and verbal communication.

Preferred Qualifications

Experience using Epic

Bilingual (a plus!)

Additional single-state LPN licenses

Work-from-Home Requirements

High-speed internet connection

Dedicated, HIPAA-compliant workspace

Physical Requirements

Ability to sit for extended periods

Ability to speak clearly and listen attentively over the phone

Ability to take thorough, real-time notes during patient interactions
